REGISTRATION REQUIREMENTS
In order to register for enrollment, student MUST meet the following requirements:
1. ONE OF THE FOLLOWING:
o Student lives in LAMAR Zone (Click here to check if you are zoned to Lamar HS)
o Student has an approved transfer to Lamar High School
(Students in Lee Boundary Option Zone MUST have a Transfer)
2. Student has completed or is now completing 8th grade
3. If registering to attend current school year in progress, the student must provide a completed withdrawal form
from their previous school.
4. Student can be registered ONLY BY PARENT or LEGAL GUARDIAN**
o in case of divorce, parent must have custodial rights to register student.
**Legal guardianship is established by a court.
A notarized letter does NOT suffice as proof of legal guardianship.
REQUIRED DOCUMENTS
All of the following documents are REQUIRED by the State of Texas and/or HISD. All of these documents must be
brought in at the time of registration in order to begin the registration process.
1) Government issued Photo identification of registering parent or legal guardian.
(Drivers License, State Identification, Passport, Military with the address matching the utility bill or lease)
2) Student Birth Certificate
3) Proof of Custody if applicable. (Most recent Divorce Decree concerning custody)
4) Proof of Residence
a) Recent (within 3 months) utility bill (electric, water, gas) in name of parent/guardian indicating services
address matching the identification.
b) Copy of lease listing parent/guardian as residents - address and telephone number of leasing office MUST be
provided. (The address must match the parent/guardian identification)
5) Immunization / Vaccination Record and current health insurance information.
6) School Records: From 8th grade and above (unofficial transcript reflecting all credits earned and the most recent
report card.)
ALTHOUGH YOU MAY HAVE SUBMITTED SOME OF THE ABOVE DOCUMENTS WITH APPLICATIONS TO
SCHOOL PROGRAMS, YOU MUST HAVE THE ABOVE REQUIRED DOCUMENTS WITH YOU OR YOU WILL
NOT BE ALLOWED TO REGISTER. NO EXCEPTIONS!

REQUISITOS PARA MATRICULARSE
Los estudiantes deben de tener los siguientes requisitos para matricularse:
1. Uno de Los Siguientes:
o El Estudiante vive en la zona de Lamar
o El estudiante tiene una letra aprobando la transferencia a Lamar High School
(Estudiantes en Zona Opcional de Lee a Lamar debe de tener una transferencia
aprobada)
2. El estudiante a completada o completara el octavo grado.
3. El estudiante puede ser registrado solamente por el PARIENTE ó GUARDIAN LEGAL**
o En caso de divorcio, el pariente debe tener la custodia del estudiante para registrar
al estudiante.
**La custodia legal será determinada por la corte.
Una carta notariada no es prueba suficiente para comprobar la custodia legal.
Si el pariente o guardián legal no puede atender a la matriculación del estudiante,
se permitirá una carta notariada dando a la tercera persona la autorización para
registrar al estudiante.
DOCUMENTOS NECESARIOS PARA LA MATRICULACIÓN
Los siguientes documentos son requerimientos por el Estado de Texas y/o HISD:
1. Padre o Guardián matriculando al estudiante deberá de tener una Identificación con Foto
del Gobierno
(Licencia de Conducir, Identificación del Estado, Pasaporte, Identificación Militar
conjugando el domicilio con la factura de electricidad o arrendamiento)
o Tercera persona en matriculación, se requerida una letra notariada de autorización
con la copia de la identificación del padre o guardián.
2. Prueba de la custodia del estudiante, si aplica al caso.
(Documentos de divorcio mas recientes demostrando la custodia del estudiante)
3. Prueba de Residencia
o Reciente (entre 3 meses) Factura de Electricidad. Debe de tener el nombre del
padre/guardián indicando servicios. El domicilio debe de conjugar con la
identificación.
o Copia del arrendamiento enseñando el nombre del padre/guardián – dirección y
numero de teléfono de la oficina debe de ser proveída. (El domicilio debe de
conjugar con la identificación.
o Estudiante que no viva can un padre o guardián legal no podrá matricularse asta
Agosto y necesitara la Determinación de Residencia de HISD.
4. Prueba de la Edad del Estudiante/Identificación
o Acta de Nacimiento
5. Expedientes actuales de inmunización (Vacunas) Información del seguro de salud mas reciente.
6. Cartilla Escolar
o Expediente escolar mas reciente.
Aunque usted haya entregado unos de los documentos indicados en este
documento a uno de nuestros programas educativos, usted debe de tener
los documentos requeridos en este documento CON USTED o usted no
podrá registrar al estudiante. Sin excepciones.
